EVALUATION OF ENHANCED CARE MANAGEMENT DURING PREGNANCY THROUGH PERINATAL COMMUNITY HEALTH WORKERS

Evaluate a program developed by the Prince George’s County Health Department to provide enhanced care during pregnancy through perinatal community health workers. Community health workers will identify social and clinical risks that can influence pregnancy-related outcomes. The evaluation will assess project effectiveness and sustainability and offer recommendations for expansion.
